Here are some resources I have found about Parkinson's Disease (PD) and the management of PD as a chronic disease in Canada.

University of British Columbia pairs with Pacific Parkinson's Research Institute completed a 5 year project to transform the management of Parkinson's and the development of personalized tech to monitor the disease remotely. The health report explores the provincial Initiatives in British Columbia that are established for chronic disease management. The Chronic Care Model was first published in 1998 and focused on productive interactions between the patient and the medical care practitioner with the goal to improve outcomes of the CDM. British Columbia built on the Chronic Care Model to develop their Chronic Disease Care Management. Below is an image of the Integrative Care Model.

This journal article from The Lancet Neurology proposed the integrative care model I found similar to the Social Ecological Model. According to Bastiaan, there are six barriers that need to be addressed to reach the inner circle of the integrative care model. (2020)
