Career Path

The Community Brain Injury Service Integration Project was initiated by the Trauma Network in response to the complexities surrounding the discharge of individuals with brain injuries. Our team recognizes that limitations in community-based services and barriers to access impacts individuals’ ability to fully recover and thrive in their home communities.

As the project lead I coordinate services across health, housing, justice, community partners, and both provincial and municipal sectors. My work focuses on stakeholder engagement, identifying system-level service gaps, and advancing integrated, equitable pathways for individuals with complex needs.

Clinician works closely with the multidisciplinary team providing consultation and case management to promote Independence while maximizing client's capabilities.

Collaborates with other team members on complex clients, participating in and contributing to assessments; care planning and needed follow up for the implementation and on-going evaluation of behavioural strategies, as well as engaging members of the client’s support network.

 

Assesses clients' recreation and leisure needs, develops and coordinates the delivery of therapeutic recreation plans and evaluates outcomes/objectives.

Collaborates with Occupational/ Rehabilitation staff, other health care professionals, social and recreational agencies/groups in the resolution of client program issues.
Plans, assigns, supervises and checks the work of programme staff. Interviews, hires, trains and develops staff succession plans;  conduct performance evaluations; establishes and maintains liaison with community groups and agencies.

Prepares and carries out therapeutic, recreational and social activities designed to stimulate client's physical, social and mental needs. 

Carries out individual and group therapeutic, recreational, and physical activities to improve and maintain the functional abilities of patients.

Practicing valued activities of daily living (i.e. dressing, toileting, feeding, functional transfers) with patients and cognitive rehabilitation, such as memory games, newspaper therapy. Assisting with splinting.
